As we prepare for our upcoming adventures, we need an updated BSA Annual Health and Medical Form for every Scout (and any participating adults). Please complete Parts A & B of the form and turn in one copy into your Den Leader. NOTE: If you plan to attend any overnight camping trips, please make a second copy to bring with you to camp.
